***Divisions to be offered will be determined by player registrations for each session.  Everyone who registers to play is expected to play, no matter what division they are drafted in.  Champions Field House cannot guarantee that a player will be drafted into any particular division. 

***Any player who drops out of the league after a draft has taken place based on their dissatisfaction with their division placement or team, or who drops out during the session, will not be allowed to play in future sessions.  No refunds.  No credits.

  Requests for partner picks can be made and will need to be approved by the league administrator.
  How did you hear about the league?
 

The league can accommodate up to 36 teams. The number of men versus women per team will be determined based on the total number of players registered for the league and their individual skill level. The pickup night and skill evaluation night will help the league director and team captains determine the number of teams in each division. If there are not enough captains for all of the teams in a particular division, teams may be assigned by the Champions Field House Volleyball Director and/or the “Sign Up As A Single” League Director.

IIf you have not played in this league before, you are strongly encouraged to attend the player evaluation night as well as provide as much information as possible about your skill level and experience on this registration form (high school/college experience, other leagues you’ve played in and at what level, tournaments, etc.). By providing this information and participating in the skill evaluation night, you will improve your chances of being drafted in a division suitable to your experience and skill.



Amateur Minor Athletic Waiver and Release of Liability

In consideration of being allowed to participate in any way in the Champions Fields House athletics/sports program and related sports and activities the undersigned:

 

RELEASE OF LIABILITY, AGREEMENT TO ARBITRATE—READ BEFORE SIGNING
INDEMNITY: PLAYERS SHALL PLAY AT THEIR OWN RISK. ANY BODILY INJURY OR DAMAGE TO PROPERTY OF OTHERS CAUSED BY CUSTOMER’S USE OF CHAMPIONS FIELD HOUSE IS THE SOLE RESPONSIBILITY OF THE CUSTOMER. AS AN EXPRESS CONDITION FOR RENTING OR UTILIZING THE DESIGNATED SPACE, THE CUSTOMER AGREES TO INDEMNIFY AND HOLD HARMLESS CFH, ITS OFFICERS, AGENTS, EMPLOYEES, OWNERS AND CONTRACTORS, FOR ANY AND ALL LOSSES, INJURIES, OR DAMAGES TO ANY PERSON OR THING THAT SHALL ARISE FROM CUSTOMER’S USE OF CHAMPIONS FIELD HOUSE.

In consideration of being allowed to utilize the CFH, its related events and activities, I, the undersigned, acknowledge, appreciate, and agree that:

  I, for myself and as agent for those who will utilize the space which I have rented and/or any user or participant in any related CFH clinic, practice, league or other event HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE CFH, their officers, official agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity (“Releasees”),  WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM MY OWN NEGLIGENCE OR OTHERWISE to the fullest extent permitted by law.
  I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE RELEASEES or others, and assume full responsibility for my OR MY CHILD’S participation; and,
  ARBITRATION AND CLASS ACTION RESTRICTION: Customer hereby agrees to resolve any and all disputes arising out of this contract which through binding arbitration and further to handle each potential claim on an individual basis and not through class action. Any controversy less than $2500 shall be limited to 2 hours or less by the ADR Representative. Said individual binding arbitration shall be administered by and under the rules of the “ADRMARYLAND” www.adrmaryland.com 1-888-88ADRMD under its Arbitration Rules and in Montgomery County, Maryland. Judgment on the award rendered by the Arbitrator(s) may be entered in any court having jurisdiction thereof. By agreeing to this the decision of the arbitrator is final the parties understand that they each waive their right to have such disputes resolved by trial by jury. The costs of arbitration shall be equally split by the parties pursuant to the rules of ADR Maryland.
